In order to help you save time in our office, you now have the option to fill out our forms before you’re scheduled to come see us. Simply click on the link below to download and complete the forms at your leisure. When the day of your appointment arrives, just bring the forms with you, and we’ll get you in to see Dr. Voto right away. Please let us know if you have any issues accessing our forms.
We accept most dental insurances, and we will file the paperwork on your behalf. You are responsible for copayments, deductibles, and remaining balances. To discuss your dental plan benefits, please call our office.
If insurance won’t cover the full expense of a procedure, or if you’re a self-pay patient, then you may qualify for special financing with CareCredit.
We also accept cash, personal checks, Visa, MasterCard, Discover, and American Express.
